What are the requirements for proper documentation?
WSU/DSS Documentation Policy of a Learning Disability
- Must have been completed within the past three years
- Diagnostic interview
- Assessment: neuropsychological or psychoeducational evaluation
- Aptitude/Cognitive Ability (intellectual assessment: WAIS-3)
- Academic Achievement (SATA or WIAT or Woodcock Johnson)
- Information Processing (subtests on WAIS-3 or Woodcock Johnson)
- Personality Test (MMPI-2)
- Documentation must include a specific diagnosis from the DSM-IV
- Recommended services by the evaluator must include a rationale

- Behavioral rating scales of ADHD
- Personality Test (MMPI-2)
- Identification of DSM-IV criteria
- Recommended services by the evaluator
